As part of our continuing effort to improve the health and health care of Coloradans, the Colorado Health Foundation is conducting brief surveys with diverse stakeholders to help us evaluate the effectiveness of our quarterly magazine, Health Elevations and monthly email update, The Progress Report. Both of these publications highlight programs, policy issues and events related to our mission to make Colorado the healthiest state in the nation. We know that garnering feedback and insights in this manner is one of the best ways to help ensure that our communications are valued and continue to meet the needs of our community.

We greatly value your insights and would appreciate your taking the time to complete a short electronic questionnaire. The survey should take approximately 10-15 minutes to complete. We would like to get your response by April 19.
